Lodi, CA 95242 Revised SR0061692 <br /> The San Joaquin County Environmental Health Department(EHD) has completed the review of <br /> the application for a permit to replace the penetration boots at UDC 13, 14/15, 16/17, 20 main <br /> 21 main, 21 satellite, replace the 91 fill bucket, and replace torn secondary test boots at UDC <br /> 13, 16, 28, 21, and #3 diesel sump on the underground storage tank(UST) system at the above <br /> noted address. An addendum has been added to include replacing the Western Fiberglass Co- <br /> Flex pipe between diesel dispenser 20 &21 with new Western Fiberglass Co-Flex pipe. The <br /> permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Aris Cacapit at(209) 468-3453 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. Corrosion protection: Verify corrosion protection for direct bury spill bucket prior to <br /> backfilling. <br /> b. Spill bucket test: Verify minimum 5-gallon capacity and pass a one hour hydrostatic <br /> test(or equivalent). Proper drainage must also be verified. <br /> c. Primary piping soap test: One hour pneumatic pressure test at 110% of operating <br /> pressure, or 40 psi, whichever is greater. <br /> d. Secondary piping soap test: One hour pressure test per manufacturer's guidelines. If <br /> there are no manufacturer's guidelines, test shall be performed using an applicable <br /> method specified in an industry code or engineering standard. <br /> e. 24-hour hydrostatic test(or equivalent) on UDC 13, 14/15, 16/17, 20 main, 21 main, <br /> and 21 satellite. <br /> 4. Since concrete will be broken near the 91 UST and diesel dispensers 20 and 21 a <br /> secondary containment test must be performed on the 91 UST and diesel dispensers 20 <br /> and 21 once concrete is poured in accordance with the manufacturer's guidelines. <br />
