0 0 <br />UST SYSTEM RETROFIT OR REPAIR <br />(Submit minimum of 3 sets of plans & applications as originals will be retained by END) <br />1. Site map enclosed? YES [4 NO [ j <br />2. Submit copies of ICC Service Technician and/or Installer's certificate and all manufacturer training certificates for <br />each person installing or testing any component that Is repaired or replaced. Ensure a copy of the "Site Health <br />and Safety Plan" is available on the jobsite as required by Title 8. <br />3. Detailed description of work to be completed. List components to be repaired or replaced and attach a diagram <br />drawn to scale showing location of repairs and/or replacements. If repairing a component, describe how this will <br />be done. (if adding piping, UDC's, or other UST equipment, or performing tank top upgrade, use the UST <br />Installation Application pages 4-8 as necessary for a timely plan review): <br />Auto Dispensers: Repair Premium secondary run from #12 to #13 due to failed pressure test and damaged test boot on #13. <br />Repair #13,14/15 and 16/17 due to failed hydrostatic test. Repair unleaded 87 secondary from #15-16 due to failed pressure <br />test and damaged test boot on #16. Repair 91 Fill Bucket due to failed hydro test (Phil-Tite). <br />Diesel Dispensers: Repair secondary run from transition sump #2-28 due to failed pressure test and tom 2" test boot on #28. <br />Repair #21 Main due to failed hydro test. Repair Secondary run from #20-21 due to failed pressure test. Repair #21 SAT due to <br />failed hydro test and tam 2" penetration boot. Repair Secondary run from Tanks #1,2,3 to Transition sump #1 due to failed <br />pressure test and damaged test boot in sump #3. Repair Tank #3 due to failed hydro test and damaged 4" test boot. <br />*Site Map, Test Report, Test Tapes attached* Investigate secondary leak In diesel product line between dispensers 20/21. <br />As necessary break concrete and expose existing product line. Remove and replace the existing Western Fiberglass Co -Flex <br />product piping product between dispenser 20/21. Perform all Inspections required by San Joaquin County CUPA. Break out <br />concrete at dispensers 20/21 cut out a section of the Western Fiberglass UDC's to have access to make repairs to the UDC's. <br />After repairs are complete re -fiberglass the UDC's in accordance with Western Fiberglass practices and perform hydrostatic <br />test. <br />4. List of equipment to be used (Attach manufacturer's specification sheets showing third -party approval): <br />Fiberglass matting and resin. <br />3 <br />
