SAN" J D�l+ �� I 1411" Environmental Health Department <br /> COUNTY�l <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Super Store Industries-Grocery Division 1 16888 McKinley Ave Lathrop Aril 29 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> Based on the following types of violations observed, the facility employees are not trained on proper hazardous waste <br /> (HW)handling or the training provided has been inadequate: <br /> -Adjacent the 55-gallon steel drum of used oily absorbent solids, located at the fuel island, a poly container labeled <br /> "Trash"was found also containing used oily absorbent solids(IMG 5718-5722). <br /> -A 55-gallon hazardous waste poly drum containing used oil was found open with a plastic drain pan on top of the <br /> drum, located in the outdoor chemical storage area (IMG 5675-8). <br /> -A 55-gallon hazardous waste steel drum containing oily absorbent solids, located at the fuel island,was found open <br /> with the lid sitting on top of the container, rather than sealed closed (IMG 5718-20). <br /> -One 55-gallon hazardous waste steel drum,found in the warehouse maintenance shop, containing paint waste <br /> liquids had an accumulation start date of 9/24/18(stored on site>180 days)(IMG 5651, 5653). <br /> -Two 55-gallon hazardous waste poly drums,found in the warehouse maintenance shop, containing used oil were <br /> each labeled with two different accumulation start dates, 11/12/16 and 10/31/18(IMG 5648-9, 5652). Based on facility <br /> manifests reviewed at the time of inspection, used oil was last picked up from the facility on 4/5/19. <br /> -An open 55-gallon hazardous waste poly drum containing used oil (with the drain pan sitting on top of the drum), <br /> located in the outdoor chemical storage area,was labeled with two different accumulation start dates, 2/6/18 and <br /> 10/31/18(IMG 5675-8). Based on facility manifests reviewed at the time of inspection, used oil was last picked up <br /> from the facility on 4/5/19. <br /> -A 55-gallon blue hazardous waste poly drum containing used oil, located in the outdoor chemical storage area,was <br /> labeled with two different accumulation start dates,4/26/18 and 4/24/19 (IMG 5682-3). Based on facility manifests <br /> reviewed at the time of inspection, used oil was last picked up from the facility on 4/5/19. <br /> -A 55-gallon steel hazardous waste drum containing used oil, located in the outdoor chemical storage area,was <br /> initially thought to be Hydrotex natural gas product engine oil since the drum was labeled as such (IMG 5684-6). <br /> -Jamie Lima, Super Store Industries Environmental Specialist, stated she did not have employee hazardous waste <br /> training records available for review at the time of inspection. <br /> The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper waste <br /> handling and emergency procedures. <br /> Provide proof of training and training materials to the EHD for employees whose responsibilities include hazardous <br /> waste. <br /> This is a minor violation. <br /> FA0003833 PRO514244 SCO01 04/29/2019 <br /> EHD 22-01 Rev.09/27/2018 Page 7 of 14 Small Quantity Hazardous Waste Generator OIR <br /> 1868 E.
