SANJ O A Q U I N Environmental Health Department <br /> --Cr O U NJ Y--- <br /> Complaint <br /> -Complaint Record Daily Time and Activity Summary from 6/5/2017 to <br /> 6/5/2020 for COON4AXUL located at <br /> MEDIAN STRIP NB 1-5 AT ELEVENTH ST <br /> OVERCROSSING <br /> Activity Date Recorded by <br /> 07/03/2019 Received a page from SJ General at 1:24 a.m. Called and received information PE 2546 ALANIZ <br /> 180 min about a spill that occurred at 1:00 a.m. Approximately 30 gallons of diesel was SC 974 <br /> released from a crash on 1-5 at the Eleventh Street overpass in Tracy. The spill <br /> was reported by CHP. Called the number received (209-943-8607) several times <br /> and received no answer. Called the hospital to make sure the number was <br /> correct. They confirmed I had been dialing the correct number. Called Juan <br /> Armendariz, Caltrans Hazmat, to see if he was at the site yet(209-483-3088). <br /> The call went to voicemail. Called and updated Muniappa Naidu, Program <br /> Coordinator. He recommended I try calling the main dispatch number. Called the <br /> main dispatch number at 209-943-8600 and got in contact with a dispatch <br /> operator. He confirmed what I had been told earlier and said that Caltrans was <br /> on their way to the incident. Updated Muniappa Naidu. Received a call from <br /> Armendarez informing me that he was on his way to the site. Left to the site at <br /> 2:10 a.m. and arrived at 2:52 a.m. <br /> On site was Armendarez and CHP Officer Daniel Garza. A truck carrying broccoli <br /> crashed into the pillar(going north bound) that holds up the Eleventh St overpass <br /> over 1-5. Diesel staining was observed on the ground where the semi-truck <br /> crashed. A 150 gallon diesel tank on the passenger side was approximately% <br /> full. The fire department contained the release from the passenger side tank by <br /> plugging the hole. The tank, however, still had a slow drip. Due to the debris from <br /> the crash, it was unknown how far the release had spread. No waterways or <br /> storm drains were impacted. I was not able to write the notice to abate. The <br /> driver died during the crash and the Sheriff's Office needed to notify the family <br /> before the responsible party could be called. The responsible party is going to be <br /> On Track Transportation Inc. The corporate phone number for On Track is <br /> 1-800-338-3515. Left site at 3:30 a.m. and arrived back home at 4:18 a.m. <br /> Note: All Star Towing was the company hired to take the semi-truck away. Told <br /> both Armendarez and Officer Garza that the towing company could not take any <br /> hazardous waste with them. <br /> Page 1 of 5 <br />
