SANJ O A Q U IN Environmental Health Department <br /> Complaint Record Daily Time and Activity Summary from 6/5/2017 to <br /> 6/5/2020 for COON4AXUL located at <br /> MEDIAN STRIP NB 1-5 AT ELEVENTH ST <br /> OVERCROSSING <br /> Activity Date Recorded by <br /> 07/03/2019 Called Juan Armendarez, Caltrans Hazmat, 209-483-3088, at 8:00 a.m., and PE 2546 ALANIZ <br /> 186 min asked for an update. He had gotten in contact with the responsible party (RP) and SC 544 <br /> gave me their phone number, 209-948-9100. At this time, the RP was currently <br /> looking for a contractor. Called the RP and spoke with Julie Jimenez. Jimenez <br /> works in the safety department. I informed her that I was going to send her a <br /> notice to abate and discussed what she needed to do. I also discussed billing and <br /> received her mailing address. It is 2701 E Hammer Ln #112 Stockton, CA 95210. <br /> She asked me to email the notice to abate to her at <br /> <jjimenez@ontracktransoffl.com>, and Katie Hachman, Management, at <br /> <khachman @ontracktransoffl.com>. <br /> Created a complaint. Emailed the Notice to Abate to her, Hachman, and <br /> Armendarez at<juan.armendarez@dot.ca.gov> saying: Attached is the notice to <br /> abate. On Track Transportation needs to immediately hire out a contractor that is <br /> approved by Caltrans to clean up the release. There is a Caltrans approved <br /> contractor list I can give you if you still need it. <br /> EHD needs to be on site to witness the final cleanup and any sampling that might <br /> occur for waste profiling. Once the cleanup has been completed, I will need to <br /> receive the final disposal records and any waste profile documentation. It usually <br /> takes a couple of weeks for you to get that record. After that, the incident will be <br /> closed with the county. As I mentioned over the phone, the EHD does bill for time <br /> spent towards this until the incident has been closed. We bill at 152 dollars/hour. <br /> She responded back asking for the list. Emailed it to her. <br /> Called Armendarez several times asking for an update. The spill was revised to <br /> 120 gallons of diesel-not including any oil/coolant that might have leaked. <br /> Received an email update email from him at 10:37 a.m. saying: Just received a <br /> call from Ray at Clean Harbors, (310)321-8503. They are getting a crew together <br /> to come out and cover the contaminated area and mark it for USA Dig. Once they <br /> confirm the location of any utilities, we will schedule a date for the cleanup. I will <br /> everyone updated. <br /> Updated Muniappa Naidu, Program Coordinator, and Haza Saeed, C person. <br /> Finished complaint information: ER Record, Prop 65....etc. Typed up narratives. <br /> Called Ray at Clean Harbors, (310) 321-8503. A voice message was left. <br /> Received a call from Armendarez letting me know that Clean Harbors came, <br /> marked the site for USA Dig, and covered the impacted area with Visqueen. The <br /> cleanup is scheduled to take place on July 10th or 11th. <br /> Received a call from the Clean Harbors Supervisor, Scott, (707)410-8242. Called <br /> him back and discussed the cleanup. He told me what Armedarez had told me. <br /> 07/05/2019 Downloaded photos and completed a photo log. PE 2546 ALANIZ <br /> 30 min Created a file for the complaint.
