E.Training and Drills Version :27.10.4 <br /> E.5 AFTER ACTION REPORT PROCEDURE <br /> This section provides procedures and information useful to responders for post <br /> incident/exercise review and evaluation. Post incident/exercise reviews should be conducted <br /> in a timely manner following an incident/exercise. The Plan should be evaluated to determine <br /> its usefulness during the incident/exercise and appropriate revisions should be made. All <br /> incident/exercise documentation should be included in the Plan evaluation process. <br /> Attendees <br /> The following individuals should be in attendance at the Critique, as appropriate. <br /> 1. Emergency Response Coordinator <br /> 2. Incident Commander <br /> 3. Section Chiefs/Leaders <br /> 4. Safety Officers <br /> 5. Participating Managers/Supervisors <br /> 6. Representative from Environmental <br /> 7. Response Team Members <br /> 8. Designated Scribe <br /> 9. Facilitator/Discussion Leader <br /> (To be appointed by the Emergency Response Coordinator) <br /> Critique Tracking Number <br /> 1. A Critique Tracking Number must be obtained from the Safety Department Incident <br /> Investigation Tracker. <br /> 2. Once the Critique is completed, it will be circulated through the Managers for review <br /> and sign off. <br /> 3. Critiques will be posted for review. <br /> 4. All Critiques will be filed in the Safety Office by the Emergency Response Coordinator. <br /> Agenda for Critique <br /> The Critique should be considered by the following agenda. <br /> Specific follow-up questions are listed following this agenda. <br /> Notification <br /> • Immediate area of the emergency <br /> . Total Facility <br /> . Community (as appropriate to the incident; could include notification of the appropriate <br /> agencies) <br /> .
