Within 15 days after the incident, he must <br /> submit a written report on the incident to the Regional <br /> Administrator. The report must include: <br /> 0)(1) Name, address, and telephone number of the owner or operator; § 2.2 - External <br /> Notifications <br /> 0)(2) Name, address, and telephone number of the facility; § 2.2 - External <br /> Notifications <br /> 0)(3) Date, time, and type of incident (e.g., fire, explosion); § 2.2 - External <br /> Notifications <br /> 0)(4) Name and quantity of material(s) involved; § 2.2 - External <br /> Notifications <br /> 0)(5) The extent of injuries, if any; § 2.2 - External <br /> Notifications <br /> 0)(6) An assessment of actual or potential hazards to human health § 2.2 - External <br /> or the environment, where this is applicable; and Notifications <br /> 0)(7) Estimated quantity and disposition of recovered material that § 2.2 - External <br /> resulted from the incident.
