0 <br /> TERMS OF DELIVERY OF MEDICAL WASTE TO BMS <br /> The MWMA states, "the CUSTOMER is solely responsible for properly and lawfully packaging and storing of their <br /> RMW-Regulated Medical Waste according to State of California Health and Safety Code Chapter 9,Section 118280"- <br /> Containment & Storage. The Customer shall/can provide, or upon request BMS will make available suitable <br /> containers for the proper removal, transportation and disposal of Medical Waste for scheduled services — for ON- <br /> CALL Service there is a nominal one-time set-up charge of$25-35.00 for this service.BMS shall have the right but not <br /> obligated: to inspect, sample analyze or test any medical waste readied for pick-up. Improperly packaged, leaking, <br /> overweight (over 55 lbs.) Medical Waste is subject to rejection, return, or to special handling charges - the <br /> "CUSTOMER"will be advised ahead of time if such charges are applicable when possible. Please remember RWM is <br /> subject to rejection, return to the "CUSTOMER" by the Transporter, Transfer Station or Treatment Facility, if the <br /> RMW is out of compliance to the MWMA. <br /> No `Bed-Bag"medical waste will be picked up that is wet or leaking-liquids must be solidified or in a leak proof collector. <br /> TERM—THIS IS A MONTH-TO-MONTH SERVICE AGREEMENT(SA)- <br /> This MONTH-TO-MONTH Service Agreement (SA) becomes effective upon initialing page 1 & 2 and signing the <br /> bottom of the page 3,the SA continues to renew automatically from month-to-month until terminated,with or without <br /> cause by either party. <br /> BILLING <br /> ON-CALL CUSTOMER's must pay by Credit Card or Check at time of pick-up. <br /> SCHEDULED CUSTOMER's may choose to be invoiced for services or supplies.Invoices shall detail: <br /> 1.) Type; 2.) Quantity; 3.) Any supplies delivered or ordered for purchase. All invoices are due and payable upon <br /> receipt;and no later than 30 days.4.)All invoices not paid within 30 days are subject to a finance of 1.5%per month,or <br /> any partial month until paid. <br /> NOT READY FOR PICK-UP <br /> A "No Access" charge of$35.00 could be imposed: 1.) after waiting a reasonable period of time (over 15 minutes), <br /> your medical waste is NOT available and ready for pick-up—the driver may have to leave to keep to schedule without <br /> completing your pick-up;or 2.) after waiting a reasonable period of time (over 15 minutes),no one is available to grant <br /> access—Barnett will always endeavor to gain access during business hours posted onyour Service Agreement;this is why it is so important <br /> to insureyour business hours are properly noted at the top of the SA—LATPORTANT—please indicate lunch hours if closed for lunch. <br /> SERVICE PRICES - <br /> See your Price List for ON-CALL service;or your Price Quote for Scheduled Services <br /> NOTE:Barnett endeavors to keep stabilized pricing for a period of time of at least two years—price lock.
