The facilities client consent form and medical questionnaire must include all of the information listed in Form B, and are <br />available for inspection. HSC 119303 <br />1. Client records were not available for all artists. Records were available for Christopher Flores for the 2018 <br />year and a couple of records from January 2019. More recent record lacked lot numbers for the <br />equipment used. Records are to be made available for all artist by 11/14/2019. Effective immediately, <br />record the lot numbers for equipment used. <br />35. Operation and employee training records shall be maintained for 3 years and made available for inspection. A written <br />Infection Prevention and Control Plan (IPCP) shall be maintained, followed and updated. HSC 119313 <br />The IPCP and training records were not available at the time of the inspection. <br />All corrections are due by 11/14/2019. <br />A re -inspection charged at $152 will occur on or after 11/14/2019. <br />Notes: <br />The facility should have the permit changed to 4121 to reflect the autoclave in use in the facility. <br />Hot water was available. <br />3. Trash cans in the procedure areas were lined. <br />
