0 Spore Test Checklist ❑ Verified 3M Attest Auto Reader is plugged in for at least 30 minutes prior to start of test ❑ Obtain biological indicators ❑ Don PPE ❑ Verified test pack and control tubes are from the same manufacturer ❑ Verified all biological indicator tubes are not expired ❑ Test pack loaded in the center of the medical waste load ❑ Place the "Test in Progress, Do Not Operate" sign on the chamber ❑ Ran sterilization cycle, verifed sterilization cycle at 250de revs F for 30 minutes ❑ Load was cooled and test pack was removed wearing PPE ❑ The medical waste was left in the chamber ❑ The incubation/readout procedure was followed: Don PPE Open test pack and remove indicator tube Close the biological cap by pressing down Crush the glass tube in the crushing well vial Place the tube into the incubator well Cover and close, wait for results (at least 3 hours) ❑ Results read Negative results: Control tube will be red, sterilized tube will be green Positive results: Control tube will be red, sterilized tube will be red Positive results: Control tube will be green, sterilized tube will be red Positive results: Control tube will be green, sterilized tube will be green ❑ Test is negative; documented all information in the Record Log Book, including: Date Load number Sterilizer number Load contents Tube Lot number/expiration date Time and initials Chemical indicator Incubator well numbers, time in/time out, initials Check "BI" box positive or negative under Visual ❑ Test is negative; Notified EVS can use the Sanipak and removed sign ❑ Test is positive; test will be repeated one time ❑ Test is positive; notify EH&S Manager, Facility Services Director, Chief Engineer and EVS Manager Performed by: Title: Date: _ Verified by: Title: Date: _ Sani-Pak Spore Testing Checklist Attachment A Policy Number: February 2009
