SAFETY MANAGEMENT OGRAM POLICY/PROCEDURE AUAL <br />TITLE: HAZARDOUS WASTE CONTINGENCY PLAN <br />PAGE 4 OF 14 <br />Acids <br />Bases & Caustics <br />Bleach <br />Flammable Liquids <br />Formaldehyde <br />Gluteraldehyde <br />Greater than 1/2 cup <br />Greater than 1/2 gallon <br />Greater than 1/2 gallon <br />Greater than 1/2 gallon <br />Greater than 3/4 cup <br />Greater than 1/2 gallon <br />Individual involved with spill will immediately call the service <br />line at 3939 to report the type of hazardous material, provide <br />an estimate of the amount spilled and the location. <br />An operator will page the Safety Manager and the Safety <br />Coordinator if either are unavailable the Emergency Planner <br />will be paged and Environmental Services Manager to verify <br />the need to contact the external contract agency. <br />If the spilled material is flammable, all ignition sources will be <br />immediately turned off. <br />In the event of personal injury, the injured party (employee) should <br />immediately report to the Employee Health department or Urgent Care <br />after 6:00 p.m. The Material Safety Data Sheet for the spilled hazardous <br />material shall be obtained and brought to Employee Health or Urgent care. <br />Visitors or Members should immediately report to Urgent Care, (Medicine <br />1). <br />Employees shall identify the material, estimate volume, and if possible <br />will isolate the spill material, provide initial neutralization. The spill area <br />will be evacuated if necessary due to potential exposure over permissible <br />exposure levels. Emergency spill response in the yellow departmental <br />Safety Management manual should be utilized to assist in spill cleanup. <br />If in a common area open to the public, staff should alert <br />patients and visitors and instruct them to evacuate or clear the <br />area. <br />Security will assist in securing the area. <br />Environmental Health and Safety Personnel <br />Fresno Revision dates: 09/12/96, 6/25/98, 3/30/00, 8/l/01,Stockton revision date 07/22/02, <br />09/19/02, 3/20/03,02/12/04, 07/06 <br />
