The medical waste management plan (MWMP) does not contain all information listed in HSC117960. <br />- MWMP shall contain all information listed in HSC 117960. HSC117960 <br />Submit a copy of the Medical Waste Management Plan updated with the following information to the EHD by <br />4/11/2019. <br />(f) The name and business address of the registered hazardous waste hauler used by the generator to have <br />untreated medical waste removed for treatment, if applicable, and, if applicable, the name and business address of <br />the common carrier transporting pharmaceutical waste pursuant to Section 118032. <br />(g) The name and business address of the offsite medical waste treatment facility to which the medical waste is <br />being hauled, if applicable. <br />(k) A statement certifying that the information provided is complete and accurate. <br />23. Biohazard bag does not meet the standard set forth in HSC 117630. <br />- Ensure that the biohazard bag is a red disposable film bag that is impervious to moisture and tear resistant. HSC 117630 <br />Biohazard waste bags used to ship waste off site must meet both the ASTM D1709 and the ASTM D1922 standards. <br />Submit evidence of correction to the EHD by 4/11/2019. <br />24. Medical, biohazardous, sharps, trace chemo, pathology, or pharmaceutical waste shall be segregated and contained <br />pursuant to this section. The container shall be properly labeled. HSC 118275 <br />Several pharmaceutical waste containers were not labeled with the date of first use. Submit evidence of an in- <br />service training on the proper labeling of pharmaceutical waste containers to the EHD by 4/1112019. <br />24. Trace chemotherapy waste is not segregated for storage. The secondary container is not labeled appropriately. <br />- Trace chemotherapy waste shall be segregated for storage, and the secondary container shall be labeled with the words <br />"Chemotherapy Waste" or "CHEMO" on the lid and sides, so as to be visible from any lateral direction. HSC 118275(a)(4) <br />24. Pharmaceutical waste is not segregated for storage. <br />- Pharmaceutical waste shall be segregated for storage in accordance with the facility's medical waste management plan. <br />Shipping container shall be in compliance with DOT and DEA. HSC 118275(a)(6) <br />Pharmaceutical waste was discarded in several sharps waste containers. Submit evidence of an in-service training <br />on the segregation of pharmaceutical waste, and solid waste from sharps waste to the EHD by 4/11/2019. <br />Reinspecion on/about: A reinspection fee of $152 per hour may be charged.
