SAN JqWUIN COUNTY PUBLIC HEALTH ORVICES 'ENVIRONMENTAL HEALTH DIVISITO MEDICAL WASTE INSPECTION REPORT FACILITY: San Joaquin General Hospital 550 Hospital Road, French Camp INSPECTION DATE: June 19, 1995 INSPECTOR: Kasey Foley, Lead Senior Registered Environmental Health Specialist The items below represent violations of the California Health and Safety Code, Division 20, Chapter 6.1, Medical Waste Management Act. Article 9. Treatment No calibration has been provided for the steam sterilizer thermometers since November 1993. The calibration is required annually with records of the calibration maintained as part of the facility's files. This is in violation of California Health and Safety Code (H&SC) section 25090(c)(2). Have proof of calibration by 7/15/95. Biological indicator is not identified on slips showing adequate sterilization conditions were attained. This is in violation of H&SC section 25090(c)(1). Provide name of biological Indicator on slips beginning Immediately. Article 8. Containment and Storage Dialysis Unit Biohazardous waste storage container was not secured so as to deny unauthorized persons entry (container was unlocked). This is in violation of HSC section 25086. Immediately, secure biohazardous waste storage container. 4th floor, 3rd floor, and 2nd floor (PCU & Med Surgery) Biohazardous waste is being accumulated in areas not secured so as to deny unauthorized persons entry (utility rooms and housekeeping rooms were unlocked). This is in violation of H&SC section 25086. Have accumulation areas secured by 7/15/95. Biohazardous waste is being accumulated in areas not properly marked with warning signs. This is in violation of H&SC section 25086. Provide proper warning signs, as specified below, by 7/15/95. The wording of warning signs shall be in English, CAUTION--BIOHAZARDOUS WASTE STORAGE AREA --UNAUTHORIZED PERSONS KEEP OUT° and in Spanish, "CUIDADO--ZONA DE RESIDUOS-- BIOLOGICOS PELIGROSOS--PROHIBIDA LA ENTADA A PERSONAS NO AUTORIZADAS" or in another language, in addition to English, determined to be appropriate. 4th floor, 3rd floor, 2nd floor (PCU & Med Surgery), and 1st floor Biohazardous waste bags are being stored on the floor or in "hamper -style" containers, not in rigid containers which are leak resistant that have tight -fitting covers. This is in violation of H&SC section 25081(b). Provide proper containers as specified in section 25081 by 7/15/95.
