SAN JqWUIN COUNTY PUBLIC HEALTH ORVICES <br />'ENVIRONMENTAL HEALTH DIVISITO <br />MEDICAL WASTE INSPECTION REPORT <br />FACILITY: San Joaquin General Hospital <br />550 Hospital Road, French Camp <br />INSPECTION DATE: June 19, 1995 <br />INSPECTOR: Kasey Foley, Lead Senior Registered Environmental Health Specialist <br />The items below represent violations of the California Health and Safety Code, Division 20, Chapter <br />6.1, Medical Waste Management Act. <br />Article 9. Treatment <br />No calibration has been provided for the steam sterilizer thermometers since November 1993. The <br />calibration is required annually with records of the calibration maintained as part of the facility's files. <br />This is in violation of California Health and Safety Code (H&SC) section 25090(c)(2). Have proof of <br />calibration by 7/15/95. <br />Biological indicator is not identified on slips showing adequate sterilization conditions were attained. <br />This is in violation of H&SC section 25090(c)(1). Provide name of biological Indicator on slips <br />beginning Immediately. <br />Article 8. Containment and Storage <br />Dialysis Unit <br />Biohazardous waste storage container was not secured so as to deny unauthorized persons entry <br />(container was unlocked). This is in violation of HSC section 25086. Immediately, secure <br />biohazardous waste storage container. <br />4th floor, 3rd floor, and 2nd floor (PCU & Med Surgery) <br />Biohazardous waste is being accumulated in areas not secured so as to deny unauthorized persons <br />entry (utility rooms and housekeeping rooms were unlocked). This is in violation of H&SC section <br />25086. Have accumulation areas secured by 7/15/95. <br />Biohazardous waste is being accumulated in areas not properly marked with warning signs. This is in <br />violation of H&SC section 25086. Provide proper warning signs, as specified below, by 7/15/95. <br />The wording of warning signs shall be in English, CAUTION--BIOHAZARDOUS WASTE STORAGE <br />AREA --UNAUTHORIZED PERSONS KEEP OUT° and in Spanish, "CUIDADO--ZONA DE RESIDUOS-- <br />BIOLOGICOS PELIGROSOS--PROHIBIDA LA ENTADA A PERSONAS NO AUTORIZADAS" or in <br />another language, in addition to English, determined to be appropriate. <br />4th floor, 3rd floor, 2nd floor (PCU & Med Surgery), and 1st floor <br />Biohazardous waste bags are being stored on the floor or in "hamper -style" containers, not in rigid <br />containers which are leak resistant that have tight -fitting covers. This is in violation of H&SC section <br />25081(b). Provide proper containers as specified in section 25081 by 7/15/95. <br />
