A ., 0 0 <br />Article 8. Containment and Storage(continued) <br />4th floor (Utility Rm & ICU), 3rd floor, 2nd floor (PCU & Med Surgery), and 1st floor <br />Biohazardous waste containers lacked proper labeling. This is in violation of H&SC section 25081(b). <br />Label all containers holding biohazardous waste with the words "Biohazardous Waste" or the <br />international biohazard symbol and the word "BION RD", on the lid and on the sides so as to be <br />visible from any lateral direction. Provide proper labeling for containers by 7/15/95. <br />Entire Facility <br />All containers holding biohazardous waste are not being thoroughly decontaminated by an approved <br />method (they are being disinfected by spraying with a product called "Ascend"). This is in violation of <br />H&SC section 25084. Immediately, properly decontaminate all containers holding blohazardous <br />waste. <br />Proper decontamination is achieved by one of the following methods: <br />a) Exposure to hot water of at least 180° F for a minimum of 15 seconds. <br />b) Exposure to chemical sanitizer by rinsing with, or immersion in, one of the following for a <br />minimum of three minutes: <br />1) Hypochlorite solution (500 ppm available chlorine). <br />2) Phenolic solution (500 ppm active agent). <br />3) Iodoform solution (100 ppm available iodine). <br />4) Quaternary ammonium solution (400 ppm active agent). <br />Ins ecte by Received by <br />IN <br />Date <br />
