This registered large quantity generator does not file a medical waste management plan (MWMP)with San Joaquin County <br /> Environmental Health Department(EHD). <br /> -A large quantity generator required to register with the EHD shall file with the EHD a MWMP on forms prescribed by the EHD. <br /> HSC 117960. <br /> The Medical Waste Management Plan requires updating with current facility information. Submit a revised copy of the <br /> Medical Waste Management Plan to the EHD by 3/5/2020. <br /> 13. This registered large medical waste generator does not maintain individual treatment records and shipping and tracking <br /> documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain individual treatment operation records, and <br /> shipping and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. HSC117975(a) <br /> Shipping and tracking records not available for the following dates: 2/12/2020, 1/8/2020 (provide white copy), 1/1/2020, <br /> 2/13/2019, 5/29/2019, 7/3/2019, 11/13/2019, 11/20/2019, 11/27/2019, 12/18/2019,the week of 12/25/2019. Submit copies of <br /> the shipping and tracking records for medical waste picked-up from the facility on the listed dates to the EHD by <br /> 3/5/2020. <br /> 23. Biohazard bag does not meet the standard set forth in HSC 117630. <br /> - Ensure that the biohazard bag is a red disposable film bag that is impervious to moisture and tear-resistant. HSC 117630 <br /> The small biohazard bags in use in the facility do not meet D.O.T. standards ASTM D-1709 and ASTM D-1922. Discard <br /> the bags to prevent use in the facility. <br /> 24. Pharmaceutical waste is not segregated for storage. <br /> - Pharmaceutical waste shall be segregated for storage in accordance with the facility's medical waste management plan. <br /> Shipping container shall be in compliance with DOT and DEA. HSC 118275(a)(6) <br /> Pharmaceutical waste was discarded in a sharps container on one of the med-pass carts. Submit evidence of in- <br /> service training on the segregation of pharmaceutical, sharps, and biohazardous waste to the EHD by 3/5/2020. <br /> 24. Medical waste is not segregated at point of generation. <br /> - Medical waste shall be contained separately from other waste at the point of origin.
