Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br /> labeled. <br /> - Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be kept <br /> clean and in good repair. Container shall be labeled with the words"Biohazardous Waste" or"BIOHAZARD" on the lid and on the <br /> sides so as to be visible from any lateral direction. HSC 118280(c) <br /> Medical waste was discarded in thee biohazardous waste containers in the designated accumulation area without an <br /> approved biohazard bag. Biohazard containers must be lined with an approved biohazard bag prior to placing sharps <br /> containers inside. Submit evidence of correction and evidence of an in-service training on the handling and storage of <br /> biohazardous waste to the EHD by 3/5/2020. <br /> 30. Designated Accumulation Area <br /> Any enclosure or designated accumulation area shall provide medical waste protection from animals and natural elements and <br /> shall not provide a breeding place or a food source for insects or rodents. HSC118310 <br /> Blood drops are on the floor of the designated accumulation area. Clean and disinfect the area, and submit <br /> photographic evidence of correction to the EHD by 3/5/2020. <br /> All corrections are due by 3/5/2020. <br /> Required photographic evidence can be submitted to the EHD at: jeaster(a�sjgov.orq <br /> There will be a $152 charge for a re-inspection that will occur on or after 3/5/2020 due to multiple repeat violations. <br /> Notes: <br /> 1) Stericycle is the licensed medical waste hauler for the facility for biohazardous and pharmaceutical waste. Stericyle <br /> began hauling waste at the facility on 5/1/2019. <br /> 2) The large biohazard bags in use in the facility meet both the ASTM D1709 and ASTM D1922 standards. <br /> 3) Assure that a biohazard bag is placed in all biohazardous waste containers prior to placing any waste in the <br /> containers. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
