POLICY <br />PROCEDURE <br />Appendix 15 <br />Appendix B <br />Medieatioia Administratido Specific Procedm-es <br />DISCO'*NITI'IUED 1IEDIC.ITIO�iS <br />When medications are discontinued by physician order, a patient is transferred <br />or discharged and does not take medications with them, or in the event of <br />patient's death, the medications are marked appropriately and destroyed. 1Vherc <br />appropriate and permitted by law, discontinued medications may be returned to <br />the 1SSUlnyg pharlllaCV. <br />If a physiciandiscontinues a medication, the order is entered into the <br />medical record. The container is then removed from the medication cart <br />along with all backup stock and medication accountability records. The <br />medication is placed in a holding area in the medication room until such <br />time as it can be processed as per facility policy. <br />2. Medications awaiting disposal or return are stored in a locked secure area <br />designated for that purpose until disposed of or picked up by pharmacy. <br />Medications should be removed from the medication cart immediately upon <br />receipt of an order to discontinue to avoid inadvertent administration. <br />3. Discontinued medications not returned to the pharmacy are destroyed in <br />accordance with destruction policies and procedures. <br />4. Discontinued medications that are unopened (e.g., unit -dose packages, <br />sealed containers) are returned to the pharmacy in accordance with the <br />medication return policy and procedure or as specified by state regulations. <br />5.
