The medication disposition log <br />or form shall contain the following information: <br />a. For the State of , the appropriate method <br />non -controlled medication destruction is as follows: <br />Transfer to a container for release to a pharmaceutical waste contractor <br />Transfer medication to trash receptacle following destruction to <br />unusable consistency. <br />*Mixing medications with an undesirable substance, such as used coffee <br />grounds or kitty litter, and putting them in impermeable, non-descript <br />containers, such as empty cans or sealable bags; will further ensure the drugs <br />are not diverted.' <br />Transfer to the toilet or hopper <br />Transfer to the sink <br />b. For the State of , these non -controlled <br />medications shall be disposed of by the nursing care center in the presence of <br />appropriately titled professionals (check appropriate line): <br />• Licensed nurse employed by the nursing care center and a pharmacist <br />• Two licensed nurses employed by the nursing care center <br />• Administrator and licensed nurse employed by the nursing care center <br />• Others as listed: <br />c.
