Section I Disposal of Medications, Syringes and Needles Page 2 of 2 <br />5.4 <br />Disposal of Medications 1 12/08 <br />Transfer to the sink <br />b. For the State of , these controlled substances <br />shall be disposed of by the nursing care center in the presence of appropriately <br />titled professionals (check appropriate line): <br />• Licensed nurse employed by the nursing care center and a pharmacist <br />• Two licensed nurses employed by the nursing care center <br />• Administrator and licensed nurse employed by the nursing care center <br />• Others as listed: <br />c. A controlled medication disposition log, or equivalent form, shall be used for <br />documentation and shall be retained as per federal privacy and state regulations. <br />This log shall contain the following information: <br />• Resident's name <br />• Medication name and strength <br />• Prescription number <br />• Quantity/amount disposed <br />• Date of disposition <br />• Signatures of the required witnesses <br />3. If a controlled medication is unused, refused by the resident or not given for any <br />reason, it cannot be returned to the container. It is destroyed as outlined above and the <br />disposal is documented on the accountability record on the line representing that dose <br />with the required signatures. This same procedure applies to unused portions of single <br />dose ampules and doses of controlled substances wasted for any reason. <br />4. Medications not listed in Schedules II, III, IV and V (non -controlled medications) <br />shall be destroyed by the nursing care center in the presence of a pharmacist or nurse, <br />and one other witness as per state regulation.
