MEDICAL WASTE MANAGEMENT PLAN (continued) <br />Page 2 of 3 <br />our medical waste disposal contractor, this includes <br />all hauling and offsite treatment: <br />BFI - Medical Waste Systems <br />Hazardous Waste Hauler #2113 <br />820 Greenville Road <br />Livermore, CA 94550 <br />Telephone: 1-800-346-1561 <br />The storage container will be identified with a <br />numerical bar code designating the generating facility <br />and labeled with the words "Infectious Waste" or the <br />international biohazard symbol and the word "Biohazard" <br />D. The storage containers will be located in chainlink <br />caged area adjacent to the facility, secured to deny <br />access to unauthorized personnel. This area will <br />display prominent warning signs in English -- "Caution <br />Infectious Waste Storage Area - Unauthorized Persons Key <br />Out", and in Spanish - "Cuidado - Zona de Residous <br />Infectados - Prohibida la Entrada a Personas No <br />Autorizadas". The area will be kept clean at all times. <br />E. The storage containers will be picked up by BFI <br />Medical Waste Systems intact as a unit two (2) times per <br />week or as often as required, but in any event, often <br />enough so as to avoid the putrefication of the waste <br />contained therein. This facility does not have a <br />Limited Quantity Hauling Exemption; all hauling and <br />disposal is done by BFI - Medical Waste Systems. <br />F. The volume of biomedical waste to be transported and <br />incinerated or autoclaved per month is estimated at <br />approximately 1500 pounds. <br />G. Each new staff member will receive an in-depth <br />orientation in the procedure for the in-house <br />collection, transportation, and/or storage of biomedical <br />waste at this facility prior to his/her first day on the <br />job. On-going in-service training will be provided by <br />the person in charge of this area. <br />H. Tracking documents for all medical wastes handled <br />and disposed of for this facility are supplied by BFI - <br />Medical Waste Systems. This documentation is kept on <br />file and is available in the General Manager's office of <br />the Artificial Kidney Medical Clinic for a minimum of <br />three (3) years. <br />
