The medical waste management plan (MWMP) does not contain all information listed in HSC <br /> 117960. <br /> -MWMP shall contain all information list 960": <br /> The Medical Waste Management Plan should be available on-site,and an updated copy of the plan must be submitted <br /> to the EHD. The Medical Waste Management Plan submitted to the EHD requires updating with the following <br /> information: <br /> (k)A statement certifying that the information provided is complete and accurate. <br /> The certification statement was unsigned (This violation was corrected during the inspection). <br /> Notes: <br /> 1.) Reviewed two years of shipping and tracking records. Stericyle is the licensed medical waste hauler for the facility. <br /> Medical waste is pick-up from the facility once per week. <br /> 2.) The Medical Waste Management Plan (MWMP)was signed and dated during the inspection, and a copy of the MWMP <br /> was provided for the department file. <br /> 3.) The biohazardous waste bags in use meet both the ASTM D1922 and the ASTM D1709 standards. <br /> 4.) The designated accumulation area is located in the northwest corner of the building. <br /> 5.) All biohazardous waste containers in use were properly labeled. <br /> 6.) Ms. Baldwin stated that during the last inspection, it was discussed whether the facility meets the requirements for a <br /> large quantity generator. I suggested that Ms. Baldwin obtain a weight report from Stericycle and submit it to the <br /> department. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
