Field Operations Policy and Procedures <br />'. <br />Example: If the removal order identifies the decedent as "John Smith" and the hospital <br />toe tag identifies the decedent as "John Smits," do not remove the decedent from the <br />place of removal until the discrepancy has been resolved. <br />2. Any identification that is attached to the decedent by the institution that has custody of <br />the decedent must remain on the decedent. <br />3.3 White Identification Band <br />Policy. A white identification band containing the required information of the decedent must be <br />affixed to all decedents at the point of initial custody (i.e. at place of removal) and at all times <br />thereafter even if the decedent already has other identification attached. The band must be <br />plastic, waterproof, tear resistant and capable of fastening in such a way that it cannot be <br />removed or fall off unless it is cut with a pair of scissors. <br />Procedure. <br />1. Ensure extra white identification bands are kept in each removal vehicle. <br />2. Use a pen with indelible ink (i.e. permanent marker) to legibly write the required <br />information (see #3) of the decedent on the white identification band. Under no <br />circumstances may the name of the decedent or other information be written on any part <br />of the decedent's body. <br />Note: Printed labels or tags are permissible as long as they meet all requirements stated <br />in the policy. <br />3.
