Field Operations Policy and Procedures <br />d. For removal of radioactive devices for a cremation case, complete the mechanical <br />and radioactive devices section on the Authorization for Cremation and Disposition <br />Form (FIN-CS023). For any other requests for removal of radioactive devices, <br />contact Legal for additional documentation. See Implanted Combustible Devices, <br />Cremation Manual. <br />2. Forward the respective authorization forms to the location or Company representative <br />performing the preparation. <br />3. The Company representative performing the preparation reviews the respective <br />authorization form prior to performing the preparation. <br />5.2 Scheduling for Preparation and Mortuary Services <br />Policy. Authorized preparations and mortuary services must be scheduled so that each can be <br />performed in time to accommodate any visual identification, visitation and/or services. <br />Procedure. <br />1. Prioritize decedents with immediate or urgent time requirements. <br />2. Communicate to all impacted locations when any significant additional preparation (e.g. <br />autopsy, reconstruction of features, discoloration, etc.) is required, and prioritize those <br />decedents in the schedule to accommodate requested transfer times. <br />5.3 Visual identification <br />Policy. A confirmation of identification for all decedents must occur with the Authorized <br />Representative of the decedent being asked to make such visual identification. If the Authorized <br />Representative is not available to make the visual identification, a family member, friend or other <br />person acquainted with the decedent may be designated by the Authorized Representative to <br />confirm the identification. For cases involving a funeral service or visitation, the visual <br />identification must occur prior to that event. For all other cases, visual identification must occur <br />prior to final disposition. A Company representative must accompany the decedent's <br />representative (i.e. next of kin, Authorized Representative, friend) for the visual identification. <br />Procedure. <br />1. Market management determines the location where visual identifications take place. <br />2. Schedule a visual identification to be limited to fifteen (15) minutes with a maximum of <br />five (5) individuals attending. <br />3.
