When a funeral service or visitation is not planned, conduct a visual identification prior to <br />final disposition. <br />A Company representative accompanies the decedent's representative for the visual <br />identification. <br />6. Once visual identification is complete, a decedent's representative signs the Visual <br />Identification Confirmation portion of the Identification Acknowledgement Form (FIN- <br />CS003) and the Funeral Home Representative signs under the Witness of the <br />Identification Procedure. <br />7. If the Authorized Representative declines to make a visual identification, allow <br />confirmation of identification through alternative methods and complete and sign the <br />Confirmation of Identification Without Viewing portion of Identification <br />Acknowledgement Form (FIN-CS003) with the Authorized Representative. <br />a. Identification by alternative methods may include recent photographs provided <br />(photographs transmitted via facsimile are not acceptable; digital photographs <br />transmitted via e-mail are acceptable) and/or detailed description of distinctive <br />physical characteristics, including any unique physical features such as scars, tattoos, <br />etc. Unless state or provincial law provides otherwise, only the Authorized <br />Representative or a person designated by the Authorized Representative may supply <br />photographs of the decedent for identification purposes. Company personnel cannot <br />photograph the decedent for these purposes. <br />b. The Company representative completing the visual identification by alternative <br />methods signs the Identification Acknowledgement Form (FIN-CS003) as the <br />Representative Confirming Identification by Alternative Methods ". <br />8. If photographs and physical descriptions are insufficient to reasonably confirm the <br />decedent's identity, have a visual identification performed by a representative of the <br />decedent before proceeding with final disposition. <br />5.4 Embalming <br />Policy. All embalming procedures must comply with all state and local laws and regulations, <br />OSHA guidelines and relevant policies applicable to the embalming process. <br />Procedure. <br />1. Verify the Authorization to Embalm Form (FIN-CS007), has been completed. <br />2. Review the Service Information Form (FIN-CS031) or Carepoint for any additional <br />preparation instructions prior to embalming. <br />3.
