0 <br />Field Operations Policy and Procedures CEO <br />2. Perform all authorized preparation procedures per the instructions on the above forms <br />(see Embalming or Minimal Preparation of Decedent for further instructions). <br />3. Immediately after preparations are complete, fill out the Case Preparation Report (FIN - <br />CSO 19), forward the completed report to the funeral home and retain a copy at the PCC. <br />5.7 Mortuary Services <br />Policy. Mortuary services (i.e. dressing, hair care, casketing, cosmetizing) must be conducted <br />according to the instructions provided by the Authorized Representative. <br />Procedure. <br />1. Document mortuary services instructions provided by the Authorized Representative on <br />the Service Information Form (FIN-CS031). <br />a. For PCCs that are on Carepoint, enter mortuary services instructions in Carepoint. <br />2. Forward the Service Information Form (FIN-CS031) to the location or Company <br />representative performing the mortuary services. <br />3. The Company representative performing the mortuary services reviews the Service <br />Information Form (FIN-CS031) for instructions. <br />a. Confirm the casket type, description and product code number matches the designated <br />casket or container ordered for the decedent. <br />b. Obtain all applicable order forms, packing slips, receipts, and return documents to the <br />ordering location for appropriate documentation and retention. <br />4. Proceed with completing the mortuary services (i.e. dressing, hair care, cosmetizing). <br />Note: If, identification bands on the wrist of the decedent cannot be covered by the <br />clothing provided for visitation, remove the identification bands and reattach them using <br />a cable tie to the decedent's left ankle. <br />5. Place the decedent in the selected casket or container, ensuring that the decedent is <br />positioned appropriately. (See Lifting for more information on the correct and safe <br />method for placing decedents.) <br />d. Ensure that any applied cosmetics do not stain the decedent's clothing or the <br />casket/cremation container lining. <br />e.
