Field Operations Policy and Procedures
5.8 Collecting Fingerprints
Policy. Collection of a decedent's fingerprints or footprints ("samples") is allowed only for
purposes of creating keepsake articles (i.e. jewelry, card) and must be authorized by the
Authorized Representative. After collection, such samples will be temporarily secured at the
funeral home in the decedent's file and will be released for no other purpose.
Note: Company representatives are not trained nor authorized to collect samples (prints, locks
of hair, saliva, etc.) for DNA purposes. Families must contact a third party source for such
requests and arrangements will be made to have the decedent available (with the Authorized
Representative's permission) to the third party source for collection of samples. Contact the
Legal Department for any questions.
Procedure.
1. Explain to the Authorized Representative that the collection of one or more samples from
the decedent can be done but only for the purpose of creating keepsake articles such as
jewelry or commemorative cards.
2. Explain that one or more of the following conditions may apply to collection of samples:
a. In some cases, the condition of the decedent may make it difficult or impossible to
collect such samples.
b. Tissue -enhancing chemicals may be used to facilitate the collection of samples.
3. Complete the Authorization to Collect Fingerprint Form (FIN-CS099) and have it signed
by the Authorized Representative.
a. If the Authorized Representative is not sure if they want to purchase a keepsake for
samples at this time, check mark the box "Collect Sample from Decedent and
temporarily hold for later use by Vendor" and check the box corresponding to the
sample to be collected and have the form signed by the Authorized Representative.
4. Forward the Authorization to Collect Fingerprint Form (FIN-CS099) to the appropriate
personnel at the funeral home or PCC for collection of the samples.
5. Collect the authorized samples from the decedent.
6. After collection, make a copy of the samples.
7. Transfer custody of the original samples collected only to the authorized vendor and
forward the copy of said samples to the funeral home for temporary storage.
8. After keepsake articles are created, secure them until the Authorized Representative can
pick them up.
Version 2.1 Page 25 of 70 Revised: 07/10/2015
Document Owner: Operations Support © 2015 SCI Funeral & Cemetery Purchasing Cooperative, Inc.
