The Authorized Representative signs the Third Party Casket Acknowledgement Form <br />(FIN-CS076) acknowledging that the Company will not be responsible for problems <br />caused by defective caskets provided by an outside source. <br />a. If the Authorized Representative refuses to sign the form, the Company representative <br />writes "DECLINED TO SIGN" on the Authorized Representative signature line. <br />2. Upon delivery, the Company representative and the delivery person sign the Received <br />section on the Third Party Casket Acknowledgement Form (FIN-CS076). <br />3. A Company representative inspects the casket for damage or defect. <br />a. If no damage or defect is detected, the funeral home or PCC representative marks the <br />"Undamaged" check box in the Condition of Casket upon Receipt at Funeral Home <br />section of the Third Party Casket Acknowledgement Form (FIN-CS076) and signs <br />and dates the form. <br />b. If damage and/or defect is detected, the Company representative: <br />i. Checks the "Damaged" or "Defective" check box, notes the damage/defect in the <br />Condition of Casket upon Receipt at Funeral Home section of the Third Party <br />Casket Acknowledgement Form (FIN-CS076), and signs and dates the form. <br />ii. Notifies the Authorized Representative immediately of the condition of the <br />casket, and has him or her sign in acknowledgement of the information as soon as <br />possible. <br />6.4 Ceremonial Caskets <br />Policy. When a ceremonial casket is requested, the Company representative must inform the <br />Authorized Representative that the ceremonial casket (or shell) has been previously used. <br />Procedure. <br />1. If an Authorized Representative requests the use of a ceremonial casket, the funeral home <br />fills out the Ceremonial Casket Acknowledgement Form (FIN-CS070) and has the <br />Authorized Representative sign it. <br />2. Prior to use, place a new insert inside the ceremonial casket. <br />3. After the visual identification/funeral service, remove the insert in which the decedent is <br />contained from the ceremonial casket shell. <br />4.
