Field Operations Policy and Procedures (fo <br />a. Record the location's name, location number and refrigeration unit identifier at the <br />top of each log. <br />Note: It is the responsibility of the Location Manager to ensure that all non -Company <br />removal associates/vendors use the Refrigeration Log (FIN-CS085). <br />2. Upon the arrival/check-in of decedents at a location refrigeration unit, place the <br />decedents in an open numbered space in the unit. <br />3. Immediately following placement of the decedents in the refrigeration unit, record the <br />below information in the Check In section on the Refrigeration Log (FIN-CS085). <br />a. Name of decedent: <br />i. Last name written first in all capital letters <br />ii. First name and middle name with only the first letter of each capitalized <br />b. Case ID number, if available <br />c. Date of birth <br />d. Date of death <br />e. Funeral home name and location number <br />f. Date and time checked in <br />g. Name of Company representative checking in the decedents <br />h. The space number in the refrigeration unit where the decedents are placed, using the <br />first cell of "Space Nbr." on the log for the initial placement <br />i. If decedents must be moved from one space to another within the refrigeration <br />unit, write the subsequent space number in the second cell of "Space Nbr." on the <br />same line. <br />4.
