INVESTIGATION OF BODY FLUID EXPOSURES: As <br /> previously noted, all exposures (Sharp 's <br /> injuries, mucousmembrane exposure , skin <br /> exposure) must be reported . Exposures <br /> are to be reported to the Employee <br /> Accident Report Form. Reports should <br /> include the following information: <br /> 1 ) A complete description of the <br /> incident or exposure including body <br /> site on employee and mechanism of <br /> injury. <br /> 2) The patient 's body fluid involved . <br /> 3 ) The length of time of exposure if <br /> cutaneous contact with body fluids <br /> and the condition of the employee 's <br /> skin. <br /> 4) The work environment where the <br /> exposure occurred and the employee 's <br /> Job classification/department . <br /> 5 ) The immediate action taken following <br /> exposure, such as washing of the <br /> area, showering, changing employee <br /> clothing, etc . <br /> 6 ) The action taken to prevent <br /> reoccurrence or similar exposure . <br /> 7) Hepatitis B immunization status of <br /> the employee . <br /> 8 ) The involved patient 's name , and <br /> chart number. <br /> "Universal Precautions~ Taken from San Francisco General Hospital <br /> Infection Control Guidelines . <br /> Page 11 <br /> June 1991 <br />
