Document Number: 909 R 5003 <br />Procedure Rev: 12 Date: 1/22/13 <br />EMERGENCY RESPONSE PROCEDURES <br />• That employees and/or Contractors are properly trained and knowledgeable in <br />emergency response procedures. <br />• That the facility is properly supplied and prepared for an emergency response. <br />• That effective communication exists and is maintained with outside response <br />agencies. <br />NOTIFICATION <br />The following notification procedures shall be used in the event of a fire. Upon <br />receiving notification, Security shall solicit from the employee and/or Contractor: <br />• The type of emergency <br />• Location and phone extension <br />• If any employees and/or Contractors are injured and extent of <br />injuries <br />• Name of person reporting emergency <br />Once security is notified, they shall determine the severity of the emergency. <br />LEVEL 1 - Fire that involves single use of fire extinguisher or small localized <br />fire burning (i.e. trash can, small piece of equipment etc.) No personal injury <br />involved - NO 911 response required. <br />Level I requires notification to the EHS Manager, area manager, and <br />supervisor. <br />LEVEL II - Fire that involves multiple -use fire extinguisher, involves structure, <br />grass or brush fire, glass leak or any fuel sources (gasoline, propane solvents, <br />oils etc..)
