Lodi City Landfill November 16,2011 <br /> North of Awani Drive,Lodi,California Project No. 104690061 <br /> • Notify applicable regulatory agency and other personnel of the planned site investigation <br /> schedules a minimum of one week prior to conducting the fieldwork. <br /> • Obtain an approved well and boring permit from the San Joaquin County Environmental <br /> Health Department to drill and construct the LFG monitoring wells. <br /> • Stake the proposed LFG monitoring well and exploratory trench locations in the field with <br /> survey flagging and wooden stakes or spray paint based on this work plan and information <br /> obtained during the October 2011 site reconnaissance. <br /> • Notify Underground Service Alert (USA), a public utility locator, of the proposed LFG <br /> monitoring well boring and exploratory trench locations a minimum of 48 hours before field <br /> activities start so that they can contact representatives of utility companies in the area. <br /> • Arrange for and oversee a geophysical survey to evaluate potential subsurface utility con- <br /> flicts at each proposed LFG well and exploratory trench location. <br /> • Retain a qualified, experienced C-57-licensed drilling contractor to provide equipment and a <br /> 40-hour Occupational Safety and Health Administration (OSHA) trained operator to drill <br /> borings and construct LFG wells. The planned subcontractor will provide appropriate cur- <br /> rent certificates, experience, and training, relevant to the planned site investigation. They <br /> will drill approximately 10 borings and construct LFG wells in each boring to depths that <br /> correspond to the depth of wastes in that area or to depths shallower than groundwater. The <br /> LFG monitoring wells will be constructed in general accordance with applicable portions of <br /> Title 27 California Code of Regulations(CCR) for LFG monitoring programs. <br /> • Retain a licensed, qualified, and experienced backhoe contractor to provide equipment and a <br /> 40-hour Hazardous Waste Operations and Emergency Response (HAZWOPER) trained op- <br /> erator to excavate/backfill exploratory trenches and a 40-hour HAZWOPER trained <br /> technician to provide dust control. The planned subcontractor will provide appropriate cur- <br /> rent certificates,experience, and training,relevant to the planned site investigation. <br /> • Provide a California registered professional geologist, engineering geologist or engineer; or <br /> personnel experienced in such investigations and working directly under said professional, to <br /> observe and document subsurface drilling and exploratory excavations and other field activi- <br /> ties. The field personnel will observe exploratory trench excavations, spoils, and sidewalls, <br /> collect samples,and document observations and sample locations with photographs and on in- <br /> dividual trench logs. Similarly,field personnel will collect samples and document observations <br /> and sample locations with photographs and on individual boring/well construction logs.
