Self Monitoring: <br /> The results of all self monitoring programs as described in the Report of Disposal Site Information shall be reported as follows: <br /> (Vote:monitoring reports are due within 30 davs after the end of the reporting period. For example, I st quarter=January—March, the <br /> report is due by April 30,etc. Information required on an annual basis shall be submitted with the nth quarter monitoring report, unless <br /> otherwise stated.) <br /> Program Reporting Frequency Agency Reported To <br /> Maintain daily records of the types and quantities of municipal solid Quarterly Enforcement Agency <br /> waste received each day. Daily records shall be available to the EA <br /> upon request. For reporting purposes,the quarterly report shall <br /> provide, in tons,the monthly total of waste received and the peak <br /> daily load received during the quarter. <br /> Results of the hazardous waste load checking program, including Quarterly Enforcement Agency <br /> the quantities and types of hazardous wastes,medical wastes or <br /> otherwise prohibited wastes found in the waste stream and the <br /> disposition of these materials. <br /> Maintain daily records of the number and types of vehicles using the Quarterly Enforcement Agency <br /> facility per day. Daily records shall be available to the EA upon <br /> request. For reporting purposes.the quarterly report shall provide <br /> the monthly total of the number of vehicles that used the facility <br /> during the quarter. <br /> Copies of all written complaints regarding this facility and the Quarterly Enforcement Agency <br /> operator's actions taken to resolve these complaints. <br /> Results of the landfill gas monitoring program. Quarterly Enforcement Agency <br /> Wet weather preparedness report'winter operations plan. Annual—due by November 1 Enforcement Agency <br /> Fill sequencing plan for the forthcoming year.
