Self Monitoring: <br /> Results of all self-monitoring programs as described in the Report of Facility Information,will be reported as follows: <br /> Program Reporting Frequency Agency Reported To <br /> 1. The areas of the site that were used for disposal. Quarterly LEA <br /> 2. The quantities and types of wastes received each Quarterly LEA <br /> day(in tons),and the separated and recycled <br /> material(in tons)on a periodic basis. The <br /> average daily tonnage for the reporting period <br /> shall also be included. <br /> I The number of vehicles utilizing the facility per Quarterly LEA <br /> day of operation. <br /> 4. Logs and reports of all complaints regarding the Quarterly LEA <br /> facility and the operator's actions taken in <br /> response to the complaints. <br /> S. Logs and reports of all employee and customer Available Upon Request LEA <br /> injuries. <br /> 6. Logs and reports of all shutdowns other than the Quarterly LEA <br /> closed days specified on page 1 of the SWFP. <br /> Notify the LEA at least 48 hours prior to <br /> scheduled shutdowns and within one day of <br /> unscheduled shutdowns. <br /> Logs of special or unusual occurrences and the Available Upon Request LEA <br /> >perator's action(s)taken correcttmsoive the <br /> problem/situation. Special occurrences may <br /> include weather conditions that adversely affect <br /> facility operations; fires;explosions;property <br /> damage;accidents and/or injuries;any incidents <br /> involving hazardous waste or prohibited waste. <br /> 8. The quantities and types of hazardous wastes Quarterly LEA <br /> found in the waste stream and the disposition of <br /> these materials. <br /> 9. A summary of the monitoring data submitted to Semi-annually LEA <br /> the Regional Water Quality Control Board. <br /> 10.Results of the landfill decomposition gas Quarterly LEA <br /> monitoring program. <br /> 11.An employee training log with dates of training, Available Upon Request LEA <br /> course descriptions,etc.shall be maintained and <br /> kept current <br /> 12.An annual report indicating the tonnage and Annually LEA <br /> number of cubic yards of solid waste disposal <br /> capacity that was filled during the preceding <br /> calendar year,the number of cubic yards of <br /> remaining disposal capacity and map showing the <br /> areas with remaining capacity. <br /> The quarterly reports shall be submitted in <br /> accordance with the following schedule. <br /> v me Period Reoort Due <br /> Ary thru March May I <br /> April thru June August I <br /> July thru September November 1 <br /> October thru December February I <br />
