Austin Road Landfill Appendix B <br /> Contaminant Plume Characterization Report Field Procedures <br /> ■ Sample identification code <br /> ■ Date and time of sampling <br /> ■ Signature of sample collector <br /> 8.4.5 Sample Handling and Chain-of-Custody <br /> It is necessary to provide documentation of the handling of each sample from the time it is collected <br /> to the time it is discarded or archived. To maintain a record of sample collection,transfer between <br /> personnel,shipment,and receipt by the laboratory,documentation in log books and a chain-of- <br /> custody record was employed. <br /> Chain-of-custody sheets consist of multi-part carbonless forms that list the I.D.numbers of the <br /> samples in custody,time and date of sample collection,sample type,analytical parameters,and a - <br /> series of signatures documenting each time the samples are relinquished or received. Typically,a <br /> chain-of-custody sheet documents the transfer of a group of samples shipped together. The original <br /> custody record is shipped along with the samples,while the initiator of the record retains a copy. <br /> When charge of a specific group of samples changes several times,each custodian may not retain a <br /> copy of the custody record. This is acceptable as long as the original custody record indicates that <br /> each person accepting the samples has subsequently relinquished custody appropriately. Chain-of- <br /> custody forms will be completed according to the following protocol: <br /> ® The originator fills in all requested information from the sample tags. <br /> ■ The originator signs in the top left"Relinquished by"box and keeps the copy. <br /> i <br /> ■ The original record sheet is shipped with the samples. _ { <br /> ■ Therson receiving custody checks the sample tag information against the custody ; <br /> pe g <br /> record. He also checks sample condition and notes anything unusual under <br /> "Comments"on the custody form. <br /> ■ The person receiving custody signs in the adjac6nt"Received by"box and keeps the _ <br /> original. ! <br /> ■ The date/time is the same for both signatures,since custody must be transferred <br /> between two individuals. However,when samples are shipped via common carrier, <br /> the date/time is not be the same for both signatures. <br /> t When samples are shipped via common carrier,the original custody form is shipped <br /> with the samples and the shipper keeps the copy.
