LADDER INSPECTION FORM <br /> SAFETY INSPECTION FOR LADDERS-MONTHLY <br /> REGULATION: All ladders shall be maintained in good condition at all times. The use of ladders with broken or missing rungs or steps; broken or split side-rails or <br /> other faulty or defective construction are prohibited. When ladders with defects are detected, they shall be immediately withdrawn from service. Compliance with <br /> these regulations is required by Occupational Safety and Health regulations for Construction Part 1926 Subpart X. <br /> INSTRUCTION:All ladders shall be inspected monthly. Those that are discovered to have a defect shall be tagged "UNSAFE DO NOT USE"and removed from the <br /> work area for repair or destruction. Those that are in good condition shall be color coded with the safety color codes of the month (see CMS-710-02-FM-00502 <br /> Monthly Color Code). Durable color code material (i.e. colored tape, colored tie-wraps, etc.) shall be placed (one wrap) on the right side rail between the third and <br /> fourth rungs or steps. The effective date for color codes shall be the first work day of each month. Complete this inspection form by placing a check under"OK' if <br /> the ladder meets requirements of the regulation or a check under"NO"if the ladder is defective and removed from service.
