These amendments were prepared in <br />accordance with California Code of Regulations, Title 27 (27 CCR), Section 21640(b)(4) as <br />part of the Five -Year Permit Review submittal. In addition, this submittal also includes two <br />copies of a revision to the Solid Waste Facility Permit (SWFP) application form along with <br />Attachment A-2 (to the application package), which includes operating permits, as <br />requested. <br />This submittal includes the following items: <br />Solid Waste Facility Permit Application Package <br />• Revised page 3 of the SWFP application form; and <br />• New - Attachment A-2, which includes permits from the California Regional Water Quality <br />Control Board, Central Valley Region and the San Joaquin Valley Air Pollution Control <br />District. <br />Amendment No. 3: <br />• Updated cover (outer and inner) and spine; <br />• Updated Table of Contents (TOC) - pages i and ii; <br />• Amended PCPCMP Sections 1.0, 2.0 and 3.0; <br />• Table 1 - Staged Closure Matrix; <br />• Table 2 - Closure Implementation Schedule; <br />• Table 3 - Closure Cost Estimate Summary; <br />268 N.
