On November 23, 2005, the LHD issued the Notice and Order 05-01 with the compliance schedule to Forward Inc. for <br />the violations of PRC 44004 Significant Change, PRC 44104 Terms and Conditions of Permit, and T27 CCR 21600 <br />Report of Disposal Site Information. The compliance date was December 23, 2005. <br />Notice and Order 05-01 is expired. Due to non-compliance status of the facility, the issues were referred to the San <br />Joaquin County District Attorneys Office for further enforcement and resolution. <br />B. Load checking program needs to be updated to include the requirement of the removal of the universal waste. Operator <br />furnished training records for personnel for procedures of handling U -wastes. <br />C. Gas is below 5% at the boundary. 0 <br />D. Dumping pad is covered with TASW. In the CII site, the new waste is placed in the area covered with TASW, not the <br />area covered with tarps. <br />E. Per Ruben, one load of 30 cubic yard of TASW weighs 12 tons and covers 18 square feet of waste. <br />F. TASW was used as cover for the CII active face.
