Forward Landfill Inspection Report <br />November 29, 2006 <br />1. Violation T27CCR 21600 — Report of Disposal Site Information — The JTD has not been amended to <br />include the information listed in T27CCR21600 (b)(6)(B), the ADC usage and waste to cover <br />ratio. <br />a. Submit application and RDSI amendment to include all information listed in T27CCR216000 <br />(b)(6)(B) to LEA for approval. <br />b. Also see inspection dated 4/20/05, 5/25/05, 6/24/05, 7/27/05, 8/26/05, 9/29/05, 10/28/05, <br />11/28/05, 12/28/05, 1/24/06, 2/08/06, 3/30/06, 4/21/06, 5/18/06, 6/23/06, 7/28/06, 8/31/06, <br />9/29/06 and 10/27;/!)6 for previous violation of this section. <br />c. The proposed JTD amendment received on August 2nd, 2005 was rejected by this LEA in a <br />response letter dated August 23rd, 2005 due to conflicting with existing approvals for the <br />facility. <br />d. The LEA received the. proposed JTD Amendment on January 1, 2006. The LEA rejected the <br />JTD amendment on March 17, 2006 due to noncompliance with Title 27 CCR Section 21665. <br />A meeting was held with Forward Inc. on March 23, 2006 regarding this issue. <br />e. The LEA receiveda subsequent JTD Amendment on May 23, 2006. The LEA rejected the <br />subsequent amendment on June 22, 2006 because it did not meet Title 27 CCR Section 21665. <br />f. On October I I and 12 Forward Inc., CIWMB staff and EHD staff conducted an onsite ADC <br />volume/density test. The draft report of the testing was received by the EHD on November 11, <br />- 2006- T`,, -,sults will be used to determine allowable site usage of ADC for the JTD <br />amendment. <br />g.• To date, the EHD has not received any additional information regarding the JTD submittal. <br />2. AC T27CCR 20580 -Lighting — Inadequate lighting; trucks were dumping waste in the dark, the <br />portable light were not being operated at time of initial trucks at the special waste working <br />face: <br />a. Provide adequate lighting. <br />b. Ensure that all lights are functional prior to allowing trucks to dump. <br />3. AC T27CCR 20590 — Personnel Health & Safety — Several drivers were walking without hardhats <br />or safety vests in the dumping areas. <br />a. Provide adequate personnel safety equipment. <br />b. Ensure that all personnel, including truck drivers, wear safety equipment when working on <br />active face area. <br />4. AC T 27 CCR 20690 — Alternate Daily Cover — The tarps used on Class III working face had large <br />tear. The Class III working face was using Processed Green Material (PGM) to cover areas in <br />and around the tipper. The area in and around the tipper should be clean and the waste <br />removed instead of burying portions of the tipper. <br />a. Properly repair or replace the torn tarp. <br />b. Ensure that ADC is used to cover the active working face and not part of the equipment that is <br />difficult to clean or keep clean. <br />5. Violation T 27 CCR 20695 — Alternate Daily Cover - The material that is received by Forward Inc. from <br />the Stockton Recycling listed as "fines" is being claimed as "ADC Other" is not approved for <br />use as ADC. A sample of this material was supplied to the EHD and the CIWMB for purposes <br />of possible use as ADC some time in 2003. After review of the material and discussion with <br />the CIWMB staff it was determined that the material would need to have a site-specific <br />demonstration project conducted prior to approval as use for ADC. <br />a• This is not an approved material for use as ADC.
