<br /> 17. Enforcement Agency(EA)Conditions:
<br /> a. The operator shall comply with all State Minimum Standards for solid waste handling and disposal as specified inTitle 27,
<br /> California Code of Regulations.
<br /> . The operator shall maintain a log of special/unusual occurrences. This log shall include,but is not limited to,fires,explosions,
<br /> the discharge and disposition of hazardous or unpermitted wastes,and significant injuries,accidents or property damage. Each
<br /> log entry shall be accompanied by a summary of any actions taken by the operator to mitigate the occurrence. The log shall be
<br /> available to site personnel and the EA at all times.
<br /> c. Additional information concerning the design and operation of the facility shall be furnished upon request and within the time
<br /> frame specified by the EA.
<br /> d. The LEA may prohibit or condition the handling and/or disposal of solid wastes to protect the public health and safety,protect
<br /> and rehabilitate,or enhance the environment or mitigate adverse environmental impacts. The Asbestos Containing Waste
<br /> (ACW)Program implemented at this site must be conducted in accordance with Public Resources Code(PRC), Section 44820
<br /> and Title 14 CCR Sections 17897.16-17897.25 and in accordance with any and all applicable federal,state and local permits,
<br /> ordinances,regulations,and laws.
<br /> e. Site employees shall receive adequate safety training in the prevention of asbestos release to the land,air,water,and hazardous
<br /> waste recognition as well as regulatory training for asbestos handling and disposal.
<br /> f Site supervisory personnel as well as staff shall complete an OSHA—approve 24-hour Hazardous Material Awareness and
<br /> Safety Course.
<br /> g. Site personnel working in the waste receiving and disposal areas shall be trained in emergency communications. All site
<br /> personnel shall receive adequate training in operations,maintenance,and safety.
<br /> h. The site operators shall develop a comprehensive safety manual and asbestos handling and disposal manual. The manual shall be
<br /> developed and maintained for training and employee use. The manual must include the appropriate references to Title 14 CCR,
<br /> Sections 17897.00 through 17897.25.
<br /> i. The maximum permitted weekly tonnage for the combined Forward Landfill and the Forward Resource Recovery Facility is
<br /> 46,080 tons per week,and shall not receive more than this amount without a revision of this permit.
<br /> j. The maximum daily vehicle traffic allowed at the site is 620 trips per day,and the site shall not receive more than this amount
<br /> without revision of this permit.
<br /> IL Forward Landfill's contaminated soil acceptance program shall comply with the terms and conditions given in the LEA's Letters
<br /> to Forward Inc.dated October 31, 1990;June 5, 1992;and January 7, 1994.
<br /> 1. Forward Inc. Landfill shall comply with all mitigation measures specified in any certified environmental documents that are
<br /> within the authority of the LEA and are contained in a mitigation monitoring and reporting program,pursuant to PRC, Section
<br /> 21081.6. Nothing in this Solid Waste Facility Permit shall prevent the operator from complying with all other federal,state,and
<br /> local requirements. Nothing in these conditions shall be construed as relieving the operator or designee from the obligation to
<br /> obtain all required permits, licenses,approvals and/or other clearances and complying with all orders,laws,regulations,reports,
<br /> or other requirements of other regulatory agencies.
<br /> . This permit is subject to review by the EA and may be suspended,revoked,or revised at any time for sufficient cause.
<br /> n. The EA reserves the right to suspend or modify waste receiving and handling operations when deemed necessary due to an
<br /> emergency,a potential health hazard,or the creation of a public nuisance.
<br /> o. Any change that would cause the design or operation of the facility not to conform to the terms and conditions of this permit is
<br /> prohibited. Such a change may be considered a significant change,requiring a permit revision. In no case shall the operator
<br /> implement any change without first submitting a written notice of the proposed change,in the form of an RFI amendment,to the
<br /> EA at least 180 days in advance of the change.
<br /> p. A copy of this permit shall be maintained at the facility.
<br /> q. The operator is permitted to use the following materials for Alternative Daily Cover(ADC):Processed green waste, shredded
<br /> tires,pre-processed treated auto shredder waste,ash and cement kiln dust,pre-processed construction and demolition waste,and
<br /> geo-synthetic blankets. The types of ADC that do not require a demonstration project must be used in accordance with Title 27
<br /> CCR, Section 20690.
<br /> r. Pre-processed ADC materials to be used will be 18 inches or less in dimension.
