• <br />Geo-LoqkA s s o c 1 AT E S <br />CQA DAILY SUMMARY REPORT #034 <br />REPUBLIC SERVICES FORWARD LANDFILL, FU -13 <br />Location: Manteca, CA Owner: Republic Services <br />CQA Consultant: Geologic Associates Contractors: Guinn Construction <br />Prepared by: Jose Armenta (GLA) Comanco Environmental Corp. <br />Submitted to: Scott Purdy (GLA) <br />Project #: 2013-AO55 <br />Report No.: 034 Report Date: Wednesday July 3, 2013 Onsite: 06:00 - 16:30 Hrs <br />Weather: AM: 70°F Clear PM: 103°F Clear <br />Guinn Construction Activities: <br />• The Contractor continued moisture conditioning of the clay surface across the Cell floor throughout the work <br />shift. <br />• The Contractor performed moisture conditioning and work with the smooth drum roller on the clay liner <br />surface. <br />• • The Contractor performed continuous moisture conditioning of the access roads and adjacent areas for dust <br />control. <br />• The Contractor stopped work activity at 1200 hrs except for the moisture conditioning of the clay liner <br />surface. The Contractor will continue work on Monday 07/08. The Contractor will maintain the clay surface <br />throughout the weekend including the 4th of July Holiday. <br />Personnel: (1) Superintendent, (1) Grade Checker, (3) Operators, (1) Laborer. <br />Equipment: (1) 140H Cat Motor -Grader, (2) Water trucks, (1) Cat Excavator, (2) D6 Cat Dozers, (2) 637 Cat <br />Scrapers, (1) 623 Cat Scraper, (1) Cat Fork-lift. <br />Comanco Lining Activities: 0500-1630 hrs <br />• Comanco continued installation of the GCL and the single sided HDPE liners on the 3:1 slope. The Contractor <br />continued installation of the GCL and the HDPE panels from the outer edge of the anchor trench at top of <br />the slope onto the bench. The GCL panels were overlapped a minimum of 12 -inches along the length of the <br />panels with no cross seams on the slope; granular bentonite was applied on all GCL seams. The Contractor <br />covered and secured all GCL installed today with HDPE liner and ballast sandbags. <br />• The Contractor installed the HDPE liner over the approved GCL layer upon completion of a walkthrough <br />inspection with the Contractor's CQC and GLA CQA personnel. The Contractor performed pre -weld tests on <br />machine welder -technician combinations prior to the welding of the HDPE liner seams. The Contractor's <br />PAGE 1 OF 3 <br />
