Geo-Loqk <br />A S S o c I AT E SV <br />CQA DAILY SUMMARY REPORT #034 <br />REPUBLIC SERVICES FORWARD LANDFILL, FU -13 <br />construction quality control (CQC) personnel marked all panels with roll and panel numbers during <br />deployment. All HDPE liner material installed was secured with ballast sand bags at the end of the work <br />shift. <br />• Equipment: (2) Fork Lifts, (1) equipment trailer, (1) Van, (3) Trucks Personnel: 12 <br />CQA (GLA) Activities: Field Personnel (2) <br />• CQA personnel performed continuous monitoring of all construction activities insuring conformance with the <br />project specifications. <br />• CQA personnel monitored moisture conditioning and grading work being performed on the clay surface. <br />• CQA personnel performed a final walk over slope surface area intended for synthetic liner installation with <br />Guinn and Comanco personnel; the subgrade was found to be acceptable for liner installation. <br />Meetings and Site Discussions: <br />• Met with Kevin Turner (Guinn Superintendent) in the morning to discuss planned activity for the work shift. • <br />The Contractor would continue moisture conditioning of the clay surface across the entire Cell floor through <br />the weekend including the 4th of July holiday. <br />• The Contractor would observe the 4th of July Holiday from 04/07 to 07/07. <br />• Met with Jose (Comanco Superintendent) in the morning to discuss planned activity for the work shift. <br />Comanco would continue installation of the synthetic liner; would stage liner rolls on the slope and conduct a <br />walk with CQA over slope areas intended for liner installation. <br />• Comanco would continue work through the 4th and every Saturday. <br />Site Visitors: <br />• None. <br />Photos: <br />is <br />
