Geo-LoqkASSOCIATE <br />• CQA DAILY SUMMARY REPORT #041 <br />REPUBLIC SERVICES FORWARD LANDFILL, FU -13 <br />Location: Manteca, CA Owner: Republic Services <br />CQA Consultant: Geologic Associates Contractors: Guinn Construction <br />Prepared by: Jose Armenta (GLA) Comanco Environmental Corp. <br />Submitted to: Scott Purdy (GLA) <br />Project #: 2013-AO55 <br />Report No.: 041 Report Date: Thursday July 11, 2013 Onsite: 05:00 - 16:30 Hrs <br />Weather: AM: 63°F Clear PM: 94°F Clear <br />Guinn Construction Activities: <br />• Guinn Construction crew completed finish grading work in the sumps. <br />• The Contractor completed moisture conditioning of exposed clay surface today prior to the installation of the <br />double sided textured HDPE liner in the Cell floor. <br />• The Contractor performed continuous work with the smooth drum roller over the clay surface prior to the <br />liner installation. <br />• The Contractor started transferring drainage gravel material to the site; the drainage gravel material was <br />stockpiled along the NE side outside of FU -13. <br />• The Contractor performed moisture conditioning of access roads and adjacent areas. <br />Personnel: (1) Superintendent, (1) Grade Checker, (4) Operators, (2) Laborers. <br />Equipment: (1) 140H Cat Motor -Grader, (2) Water trucks, (1) Cat Excavator, (2) D6 Cat Dozers, (3) 637 Cat <br />Scrapers, (1) Cat Fork-lift, (1) 623 Cat Scraper. <br />Comanco Lining Activities: 0500-1530 hrs <br />• The Contractor started installation of the double sided textured HDPE liner over approved clay layer on the <br />Cell floor upon completion of a walkthrough inspection with the Contractor's CQC and GLA CQA personnel. <br />The Contractor performed pre -weld tests on machine welder -technician combinations prior to the welding of <br />the HDPE liner seams. The Contractor's construction quality control (CQC) personnel marked all panels with <br />roll and panel numbers during deployment. All HDPE liner material installed was secured with ballast sand <br />bags at the end of the work shift. <br />. Comanco performed non-destructive air -pressure and vacuum testing of HDPE liner seams. <br />PAGE 1 OF 3 <br />
