Geo-LoqkASSOCIATES <br />CQA DAILY SUMMARY REPORT # <br />REPUBLIC SERVICES FORWARD LANDFILL, FU -13 <br />The Contractor started installation of the 12 -oz cushion geotextile along the toe of the west and north slopes <br />underlying the Geocomposite at the toe of the slope. <br />• The geotextile panels were sewn using the double stitching sewing method. <br />• Personnel: 12 <br />• EE uipment: (1) Fork Lift, (1) equipment trailer, (1) Van, (3) Trucks <br />CQA (GLA) Activities: Field Personnel (2) <br />• CQA personnel performed continuous monitoring and documentation of all construction activities insuring <br />conformance with the project specifications. <br />CQA personnel performed a final walkthrough inspection with the liner CQC personnel over the HDPE liner <br />surface upon completing cleaning work prior to the geocomposite and the geotextile installation. <br />• CQA personnel monitored the installation of the geocomposite and the 12 -oz geotextile at all times ensuring <br />proper handling, panel orientation on the slope, tying and seaming methods, and that proper procedures• <br />were used to prevent damage to the underlying synthetic liner. <br />• CQA personnel monitored the backfill of the liner anchor trench at all times ensuring that proper procedures <br />were used to minimize and prevent damage to the adjacent synthetic liner. <br />• CQA personnel performed sampling and lab testing of granular drainage material transferred to the site. <br />Meetings and Site Discussions: <br />• Met with Kevin Turner (Guinn Construction Superintendent) and Jose Hernandez (Comanco Superintendent) <br />in the morning to discuss planned activity for the work shift. <br />• Guinn Construction would prepare clay material for clay plug and continue transferring drainage gravel <br />material to the site. <br />• Comanco would complete installation of the Geocomposite on the north 4:1 slope and would continue on the <br />west section of the 2:1 slope; would start installation of the cushion geotextile along the toe of the slope. <br />• CQA personnel attended the weekly project progress meeting held in the front entrance facility; refer to the <br />weekly project progress meeting minutes. <br />• <br />Site Visitors: <br />PAGE 2 OF 3 <br />
