Geo-LoqkASSOCIATE <br />CQA DAILY SUMMARY REPORT <br />REPUBLIC SERVICES FORWARD LANDFILL, FU -13 <br />• The Contractor continued installation of the 12 -oz cushion geotextile along the south end of the Cell floor; <br />the geotextile panels were sewn using the double stitching sewing method. <br />• Comanco performed installation of 12 -oz 18 -inch wide geotextile strips over Geocomposite cross seams on <br />the slope bench and in the NW corner of the Cell. <br />• Comanco started installation of the geogrid over approved subgrade along the west side of the top deck <br />area. The geogrid panels were overlap approximately 2 -inches and tied using white plastic ties at 2-3 foot <br />intervals. <br />• The Contractor installed the HDPE liner over approved geogrid layer upon completion of a walkthrough <br />inspection with the Contractor's CQC and GLA CQA personnel. The Contractor performed pre -weld tests on <br />machine welder -technician combinations prior to the welding of the HDPE liner seams. The Contractor's <br />construction quality control (CQC) personnel marked all panels with roll and panel numbers during <br />deployment. All DDPE liner material installed was secured with ballast sand bags at the end of the work <br />shift. <br />• Personnel: 13 <br />• Equipment: (2) Fork Lifts, (1) equipment trailer, (1) Van, (3) Trucks <br />CQA (GLA) Activities: Field Personnel (2) <br />• CQA personnel performed continuous monitoring and documentation of all construction activities insuring <br />conformance with the project specifications. <br />• CQA personnel monitored the installation of the geogrid at all times insuring proper handling and orientation <br />of the geogrid panels on the slope; performed visual inspection of the geogrid liner prior to the installation of <br />the HDPE liner insuring conformance with the project specifications. <br />• CQA personnel monitored installation of the HDPE liner at all times insuring that proper procedures were <br />used to prevent damage to the underlying geogrid material. CQA monitored and inspected all panel seams <br />for cleanliness, minimum overlap, and the implementation of proper seaming procedures. <br />CQA personnel performed a final walkthrough inspection with the liner CQC personnel over the HDPE liner <br />surface upon completing cleaning work prior to the geocomposite and the geotextile installation. <br />• CQA personnel monitored the installation of the geocomposite and the 12 -oz geotextile at all times ensuring <br />proper handling, panel orientation on the slope, tying and seaming methods, and that proper procedures <br />were used to prevent damage to the underlying synthetic liner. <br />• CQA personnel monitored the backfill of the liner anchor trench at all times ensuring that proper procedures <br />were used to minimize and prevent damage to the adjacent synthetic liner. 0 <br />PAGE 2OF3 <br />
