<br />Annual Comprehensive Site Compliance Evaluation
<br />The facility operator Shrill conduct 011e cOiiipY.ehensive 'ite
<br />compliance evaluation (evaluation) in each reporting
<br />period (.July 1 --June 30) . Evaluations shall be conducted
<br />withir, P�-16 months of each other, The StNIPPP shall bre

<br />inspection of equipment needed to iimplement the SWPPP,

<br />d. Esn `valuation report that includes, (i) id_ntificat ic,,
<br />of personnel performing the evaluation, iii) the
<br />date(s) of the evaluation, (iii) necessary SU7PPP
<br />revisions, (iv) schedule, as required in Section
<br />A.10.e, for implelitenting SWPPP revisions, ( v ) any
<br />incidents of non-con-tpliarice and the corrective actions
<br />taken, and (vi) a certification that the facility
<br />operator is in Compliance with this General Permit. If
<br />the above certification cannot be provided, explain in
<br />the evaluation report why the facility operator is not
<br />in compliance with this General Permit. The evaluation
<br />report shall be submitted as part of the annual report,
<br />retained for at least five years, and signed and
<br />certified in accordance with Standard Provisions 9. and
<br />10. of Section C. of this General Permit.
