Landfill <br />Page 2 <br />This Monitoring and Reporting Program includes a Checklist designed to facilitate verification <br />and monitoring of project compliance with required mitigation meas ues. This document will <br />be used by San Joaquin County to verify inclusion of required project design features and {/ <br />ongoing mitigation measures. The Checklist serves as a summary so that public officials, the <br />Applicant, and the public can easily determine which measures have been complied with, and <br />to what extent. <br />2.0 NUTIGATION MONITORING AND REPORTING PROGRAM CHECKLIST <br />The Mitigation Monitoring and Reporting Program Checklist is proposed for monitoring the <br />implementation of the mitigation measures contained in the Environmental Impact Report and previous <br />EIRs on the project site (see Attachment 1 for a listing of mitigation measures). The County should <br />implement the monitoring program as follows: <br />• The Director of the San Joaquin County Community Development Departr_,ent, or designee, <br />should be responsible for coordination of the monitoring program including the monitoring <br />checklist (Attachment 2). <br />• Each responsible individual or agency will be responsible for determining whether the mitigation <br />measures contained within the checklist have been complied Kith. Once all mitigation measures <br />have been complied with, the responsible individual or agency should submit a Verification <br />Report Form (Attachment 3), or similar form, and a completed checklist to the Director. <br />• If a responsible individual or agency determines that a non-compliance has occurred, a written <br />notice should be delivered to the Director describing the non-compliance and requiring <br />compliance within a specified period of time. <br />3.0 BIPLENIENTATION <br />The Director of the San Joaquin County Community Development Department would be responsible for <br />overall implementation and administration of the Mitigation Monitoring and Reporting Program Checklist <br />for the proposed Consolidated Forward Landfill Project. <br />Duties of the Director would include the following: <br />• Plan checks. <br />• Coordinate with applicable agencies that have mitigation monitoring and reporting <br />responsibilities. <br />• Assure follow-up and response to citizens' complaints. <br />• Develop forms and checklists for reporting. A sample Verification Report Form is included <br />(Attachment 3). <br />• Maintain the Mitigation Monitoring and Reporting Program Checklist or other suitable mitigation <br />compliance summary. <br />
