composite liner will exceed typical standards of practice by mandating that dedicated <br />CQA technician be available to monitor gravel and/or operations layer placement on a <br />full time basis. Further, recognizing that the greatest volume of fluids is likely to <br />accumulate in the sumps, electrostatic testing of the FML in all sump areas will be <br />completed in addition to the more conventional CQA testing protocols. <br />On the basis of the approved CQA Plan, an intensive field testing program will be <br />implemented during construction to evaluate whether the composite liner system <br />components are constructed according to the design specifications. All field tests will be <br />conducted by CQA personnel or qualified laboratories under the supervision of the CQA <br />personnel. All field observations and field testing will be recorded on inspection data <br />sheets which in combination with the technician's daily reports will be used to formulate <br />daily summary reports. All field testing will follow ASTM standard test procedures in <br />accordance with the CQA Plan. Daily inspection reports, inspection data sheets, and <br />inspection photographs will be reviewed by the CQA Manager. All inspection reports <br />will be evaluated for internal consistency, accuracy, and completeness. The above daily <br />reports will be summarized into periodic acceptance reports, which will indicate that <br />work has been completed according to the specified design. These reports will be <br />included in the project files and will be available to regulatory agencies upon request. <br />Recognizing the potential for damage to the liner during placement of the LCRS and <br />operations layers, after installation of the liner, LCRS, and operations layer, an <br />electrostatic test will be performed to identify any manufacturer- or construction -related <br />defects in discrete portions of the FML. These tests will be performed by qualified <br />is testing personnel who will report directly to the CQA Manager. As with all CQA reports, <br />the electrostatic test reports will be included in the project files and will be available to <br />regulatory agencies upon request. <br />At the completion of each phase of liner construction, a final report will be prepared to <br />include: <br />• Daily inspection summary reports. <br />• Field and laboratory test result sheets. <br />• Electrostatic FML test result sheets. <br />• Inspection data sheets. <br />• Photographic reporting data sheets. <br />• As -built reports. <br />• Deviations from design and material specifications (with justifying <br />documentation). <br />A statement that the liner has been built in general conformance with the design <br />specifications, the approved plans, and the approved modifications of the plans and <br />specifications will be provided and included in the final documentation sent to the <br />appropriate regulatory agencies.
