FIN <br />of Disposal Site Information, unless the operator believes that <br />the facility is exemptable. If so, the operator must file with <br />the enforcement agency sufficient information of the types <br />contained in a full Report of Disposal Site Information to <br />establish that an exemption p should be granted. The information <br />contained in the Report shall be used by the enforcement agency <br />to help determine conditions to be placed in the solid waste <br />facilities permit and to determine whether a permit should be <br />issued. In order to maintain the permit, the operator must file. <br />amendments to the Report whenever er necessary to keep the <br />information contained in it current. Such amendments or lack <br />therofe may become the basis for changes in the permit or for <br />revocation of the permit. A Report of Disposal Site Information <br />shall contain the following: <br />11 A descriptive statement of the manner of operation to be <br />conducted at the site. <br />The type of operation; such as daily cover, modified <br />landfill, area fill liquid q uid w aste ponds, etc., is to be <br />given as well as the site classification (class I, <br />Class Ii -I, etc.). Describe a typical operation cycle, <br />e.g., amount and frequency of compaction, daily cover depth, <br />cell size, etc. if cover material must be imported, <br />identify the daily quantity needed the source, and the <br />hauling distance. <br />
