Construction Quality Assurance(CQA) Plan Extension of Closure Cap <br /> Corral Hollow Sanitary Landfill August 21, 2008 <br /> 5.0 DOCUMENTATION <br /> The CQA Plan depends on thorough monitoring and documentation of all construction <br /> activities_ Therefore, the CQA Manager will document that all quality assurance <br /> requirements have been addressed and satisfied. Documentation will consist of daily <br /> record keeping, testing and installation reports, non-conformance reports (if necessary), <br /> progress reports, photographic records, design and specification revisions, as-built <br /> drawings, and a construction report. <br /> 5. 1 Dairy Record Keeping <br /> At a minimum, daily records will consist of a daily record of construction progress, <br /> daily construction reports, observation and test data sheets, and, as needed, non- <br /> conformance/corrective measure reports. All forms will have a peer review. <br /> 5.1.1 Daily Record of Construction Progress <br /> The dal1v field report will summarize ongoing construction and discussions with the <br /> Contractor and will be prepared by the CQA Manager and CQA Monitors. At a <br /> minimum, the report will include the following: <br /> ♦ Date, project name, project number, and location. <br /> ♦ A unique number for cross-referencing and document control. <br /> ♦ Weather data. <br /> ♦ A description of ongoing construction for the day in the area of the <br /> monitor's responsibility. <br /> ♦ An inventory of equipment utilized by the Contractor. <br /> ♦ Items of discussion and names of parties involved in discussions. <br /> ♦ A brief description of tests and observations, identified as passing or <br /> failing, or, in the event of failure, a retest. <br /> ♦ Areas of non-conformance/corrective actions, if any, (non- <br /> conformance/corrective action form to be attached). <br /> ♦ Summary of materials received and quality documentation. <br /> ♦ Follow-up information on previously reported problems or deficiencies. <br /> Vector Engineering, Inc. •143E Spring Hill Drive -Grass Valley, CA • (530) 272-2448 <br /> -26- <br />
