2.4.2 Project Control of Record Information <br /> Record (as-built) information, depending on its nature, is controlled by either the CQA Monitor <br /> or the Surveyor. As the Extension of Closure Cap of the CHLF proceeds, the CQA Monitor and <br /> the Surveyor shall route all as-built information obtained by themselves, the Contractor, and <br /> others to both the CQA Engineer and the Resident Engineer. All such as-built information shall <br /> be filed and maintained as Project Documents. When construction is complete, the Resident <br /> Engineer shall compile the as-built information and prepare Record Drawings of the work. The <br /> Record Drawings shall be included as part of the Construction Completion Report. <br /> 2.4.3 Project Control of Blank Forms <br /> The CQA Engineer controls all blank forms for daily reports, test reports, and other project <br /> purposes, and maintains a master of each form from which copies are made for project use. Upon <br /> issuance of new forms or revision of existing forms, the CQA Engineer shall recall the <br /> superseded master and all -unused copies, notify the CQA Monitor, and provide new copies for <br /> the CQA Monitor's use. <br /> 2.4.4 Daily Reports <br /> The CQA Monitor shall prepare a Daily Report of the work activities taking place each day and <br /> shall submit that Daily Report to the CQA Engineer no later than the end of the next working <br /> day. The CQA Engineer shall review Daily Reports for legibility, clarity, traceability, and <br /> completeness, and shall flag the significant occurrences and critical issues for follow-up. Each <br /> Daily Report shall be signed and dated by the CQA Monitor who prepared the report and by the <br /> CQA Engineer upon review. The CQA Monitor shall file and maintain the Daily Reports as <br /> Project Documents. <br /> 2.4.5 Test Reports <br /> ' The CQA Monitor shall complete a Test Report whenever testing is performed. This includes <br /> field tests made by the CQA Monitor, laboratory tests for which the CQA Monitor collects and <br /> processes samples, and field and laboratory tests made by others under the oversight of the CQA <br /> Manager. Test Reports shall be either peer-reviewed or reviewed by the CQA Engineer. The <br /> review shall include a check for mathematical accuracy, conformance to test requirements, <br /> conformance to construction drawings, and a check for clarity, legibility, traceability, and <br /> completeness. Each Test Report shall be signed and dated by the CQA Monitor who prepared the <br /> report and by the CQA Engineer upon review. The CQA Monitor shall transmit copies of the <br /> ' Test Reports to the CQA Engineer at the end of each week, and shall file and maintain the <br /> originals as Project Documents. <br /> CH CQA Plan 041712 Rev2a.doc 2-5 Construction Quality Assurance Plan <br />
